First Time Scheduling
Scheduling an appointment is easy. You can do it online here. Just remember to fill out the appropriate intake form, so we will know how to best help you.
Follow Up Visit
Results often require consistency. For questions about a follow up visit, or to let us know if anything has changed with your health or daily routine feel free to email Ascension at luke@ascensionclinic.com
Initial Visit
Upon arrival, plan to either have your intake paperwork filled out, or arrive 10-15 minutes early to complete it in the office. Once the intake is complete, we’ll show you where all accommodations are located throughout the office. Finally, a chiropractor will assess your nerve, spine, and muscle needs to determine your health goals and how to best serve you.
